W-9 Form Information - 9/13/2007

§         W-9 forms for deceased claimants should contain the estate information and be signed by the Personal Representative. In Part I, Taxpayer Identification Number (TIN), provide the Employer Identification Number (EIN), which is the decedent’s estate ID. If this is not available, then the decedent’s Social Security number should be entered.

 

§         Claimants that have more than one trust filing, in which all claims are linked to one another in Trust Online, only need to complete one W-9 form. The W-9 will be applied to all trusts.

 

§         Claims that are not linked in Trust Online must either have a separate W-9 completed for each trust claim or inform us in writing that you wish to apply one W-9 to all trusts for a claimant.

 

§         Law firms that would like the option to have the W-9 language inserted into some releases but submit W-9 forms for other claims should elect to insert the W-9 language for all releases. Simply submit W-9 forms for those claims that you wish and leave the W-9 language on the release blank.
